  • Consider providing your facility name and location. While not required, this supports facility evaluations.
  • Rest assured, all comments remain anonymous. We cannot identify individual respondents.

  • Click "submit" to automatically transmit your questionnaire anonymously.
Thank you for your participation!

How Magnet Uses Responses

For Organizations Preparing for a Site Visit
Comments received and attributed to an upcoming site visit are collated and sent to the appraisal team. Appraisers then amplify, verify and clarify the validity of that input during the site visit.

For Designated Magnet Organizations
Comments become part of ongoing review to spot trends. When a facility is identified, the CNO is notified of possible trends and asked to follow-up with the Magnet office to ensure the applicable Forces of Magnetism are operating effectively.
